《天下郡國利病書》，明朝地方誌書輯錄，共120卷，是明末清初顧炎武撰。在四庫全書之中為史部地理類。明亡後，顧炎武游蘇杭、兩淮，十謁明十三陵，足跡遍及全中國，旅途中以一騾二馬載著應用書籍隨行，一路「考其山川、風俗、疾苦利病」，「先取一統志，後取各省府州縣誌，後取二十一史，參互書之，幾閱志書一千餘部。」
寫成《別錄》，是一部地理志長編，晚年將此書一分為二：「一為輿地之記，一為利病之書」，第一部書名《肇域志》，第二部是《天下郡國利病書》。
天下郡國利病書重點在於政事，概分兵防、賦稅、水利三部份，以講究郡國利病貫穿全書。
梁啟超《中國近三百年學術史》中稱此書為「政治地理學」。
乾隆五十四年（1789），黃丕烈從張秋塘手中購得《天下郡國利病書》原稿。
光緒三十二年（1906）八月十五日，葉昌熾在蘇州看到吳訥士收藏的《利病書》初稿。
宣統元年（1909），方還和王頌文在吳家發現《利病書》原稿，吳訥士後人將原稿贈崑山圖書館收藏。
